What Is a Site Initiation Visit (SIV) in Clinical Trials?

Before a single patient can enroll in a clinical trial, a critical milestone must occur: the site initiation visit. For sponsors, contract research organizations (CROs), and site teams alike, the SIV signals that a site is trained, compliant, and ready to begin. Understanding what happens during this visit, and why it matters, can help trial teams protect timelines and maintain study quality from day one.

What Is a Site Initiation Visit in a Clinical Trial?

A site initiation visit is a structured, in-person or virtual meeting conducted by a sponsor’s representative, typically a clinical research associate (CRA) or monitor, at each participating clinical trial site. The SIV takes place after regulatory and institutional approvals are secured but before the site screens or enrolls any participants.

The purpose of a SIV is to confirm that every member of the site team has the knowledge, materials, and systems needed to conduct the trial in accordance with the protocol, Good Clinical Practice (GCP) guidelines, and applicable regulatory requirements.

What Happens During a Site Initiation Visit?

A well-run SIV follows a structured agenda that typically covers the following areas:

  • Protocol Review: A detailed walkthrough of the study design, eligibility criteria, visit schedule, and endpoint assessments.
  • GCP and Regulatory Training: Confirmation that the site team understands compliance requirements set by the Food and Drug Administration (FDA), the European Medicines Agency (EMA), or other relevant authorities.
  • Informed Consent Procedures: Review of the consent process, documentation standards, and participant rights.
  • Investigational Product Handling: Training on the receipt, storage, dispensing, and accountability of the study drug or device.
  • Case Report Form (CRF) Completion: Guidance on data entry, source documentation, and error correction procedures.
  • Adverse Event Reporting: Clear instruction on how to identify, document, and report adverse events within required timeframes.
  • Regulatory Binder Review: Verification that all essential documents are on file and properly organized.

By the end of the visit, the site should be fully prepared to enroll participants and manage the trial according to the sponsor’s expectations.

How Does the SIV Fit Into the Study Start-up Timeline?

The SIV is one of the final steps in study start-up. It follows site selection, IRB approval, and budget/contract finalization. However, there can be some variation regarding whether the contract needs to be fully executed by the time of the SIV and depending on site and sponsor preference as well as proximity to “final” status. Think of it as the bridge between site activation and first patient in (FPI).

  1. Site Qualification Visit
  2. IRB/IBC and Regulatory Approvals
  3. Contract and Budget FInalization
  4. Site Initiation Visit (SIV)
  5. First Patient Enrolled

Delays at any earlier stage can push the SIV back, directly affecting the FPI date. This is why optimizing the full start-up process, not just the SIV itself, is essential to keeping trials on track.

Frequently Asked Questions About Site Initiation Visits

A site qualification visit (SQV) assesses whether a site has the capabilities, resources, and patient population to participate in a trial. The SIV occurs later, after approvals are in place, to train staff and officially activate the site for enrollment.

Attendees typically include the principal investigator, sub-investigators, clinical research coordinators, pharmacists, and laboratory staff. The sponsor or CRO sends a CRA or monitor to lead the visit.

Yes. Virtual SIVs have become increasingly common, particularly for sites in remote locations. While the format differs, the agenda and objectives remain the same.

If a site can’t demonstrate readiness during the SIV, the sponsor may delay activation until outstanding issues, such as missing documents or incomplete training, are resolved.
